Complete Your Medication Administration Certification Course Application
Prior to DODD Medication Administration Certification, initial or renewal, developmental disabilities personnel must submit a completed training application to the Registered Nurse (RN) Trainer. The application must be filled in completely, including employer and personal information and required signatures.
Developmental disabilities personnel with incomplete applications are not eligible to attend DODD Medication Administration Certification training.
Mandatory Requirements Before Acceptance Include:
- High School Diploma, its equivalent, or college transcript
- Completed DODD Application Form
- Valid Photo ID (Driver’s License/ State ID/ Passport/ Permanent Resident Card)
- Enrollment Agreement – Review and Sign
- Valid Email
- CPR and First Aid Completion (5 Hour Class)
Caregivers Training reserves the right to reject applicants if the admission requirements and procedures are not successfully completed.
